In addition, a Student Verification Form [PDF document, 37k, opens in new window] should be submitted if a distribution is requested for a higher education institution and you wish VCSP to transmit the student's SSN to ensure appropriate student account processing.
The Student Verification Form authorizes VCSP to disclose the student's name and social security number to higher education institutions, if necessary, to make payment arrangements. It is our policy to transmit the student's social security number when making payments directly to a higher education institution to ensure proper credit to the students account. Therefore, both the account owner and the beneficiary must sign the form.
A separate Distribution Request Form must be completed for each individual payee. For instance, if you want us to pay tuition directly to the school and reimburse you for textbook costs, you will need to fill out two separate forms. The exact amount of the distribution desired should be indicated. Be aware that if the requested dollar amount is within five dollars ($5) of the account balance, the full balance remaining in the account will be paid to the designated payee.
VEST beneficiaries have at least ten years after the projected high school graduation date to use VEST account benefits. In addition, any years served as an active-duty member of any branch of the United States Armed Services are added to the 10-year limit. A purchaser may request an extension of the 10-year limit when the time period is about to expire. If the beneficiary has already completed high school at the time the account is opened, the 10-year period begins when the account is opened.
