	Designated Survivor	Use this form to select or change a Designated Survivor or to add or change a Successor Designated Survivor on your VPEP, VEST and/or CollegeWealth account.
	Payroll Deduction Authorization	Use this form to authorize a payroll deduction for your VPEP or VEST account.
	Rollover Investment Direction	Use this form when funding your VCSP account from existing Qualified Tuition Program or to change the portfolio selection on your VEST account.
	VPEP Down Payment	Use this form to accompany your down payment to your VPEP account.
	VPEP Intent to Enroll	Use this form to notify VCSP you are using your VPEP benefits.
	VPEP Refinance Request	Use this form to change the payment terms on your VPEP account.
	CollegeWealth Distribution Request	Use this form to request a withdrawal from your CollegeWealth account.
