Intent to Enroll form due May 19

Some prePAID account owners have an important deadline coming up. If you indicated on your prePAID application that your beneficiary would graduate from high school in 2016, complete the prePAID Intent to Enroll form and return it to Virginia529 no later than May 19, 2016. You should have received this form in the mail. You can also find it linked below.

This form serves an important purpose:

  • If you plan to use prePAID benefits this year, this form authorizes Virginia529 to communicate with and distribute payments to the eligible educational institution of choice.
  • If you do not plan to use your prePAID benefits yet, this form informs Virginia529 of that decision.

Important tips for completing the Intent to Enroll form:

  • The account owner AND the beneficiary must sign the form.
  • You MUST include the beneficiary’s student identification number assigned by the school.
  • Send in a new Intent to Enroll form if any information on a previously-submitted form changes, such as:
    • the beneficiary transfers to another school
    • the beneficiary does not attend for a semester or more
    • the beneficiary elects to use prePAID benefits after notifying Virginia529 otherwise
